Stress fractures develop when several risk factors align:

  • Rapid increase in training volume or intensity — the single biggest risk factor
  • Poor running mechanics — overstriding, excessive inward ankle roll, or weak hip stability
  • Inadequate bone density — especially in female runners with low energy availability
  • Weak calf and shin muscles — reduced capacity to absorb impact forces

  • Pain that improves with rest but returns immediately when you resume normal training
  • A gradual increase in shin pain over days or weeks (not sudden)

  • Avoid increasing pace AND distance in the same week

If you’re returning from injury or time off, be especially conservative. A runner returning after 4 weeks off should rebuild to their previous peak volume over 8–12 weeks, not 2–3 weeks.

  • Overstriding: Landing with your foot in front of your body creates a braking force and increases impact
  • Excessive pronation: Inward ankle roll that destabilises the tibia during load phase
  • Weak hip stability: Poor glute activation allows the pelvis to drop, changing tibial loading angles
  • Tight calves: Reduced ankle flexibility shifts load toward the tibia instead of distributing it through the calf muscles

Addressing these issues through gait retraining and targeted strength work reduces tibial load without sacrificing running fitness.

Strength and Conditioning for Tibial Resilience

Runners serious about stress fracture prevention should include:

  • Hip strengthening: Single-leg squats, clamshells, side lunges (2–3 times per week)
  • Calf and tibialis work: Heel walks, calf raises, eccentric calf lowering (2–3 times per week)
  • Core stability: Planks, side planks, bird dogs (2–3 times per week)
  • Plyometric preparation: Once strength baseline is built, gentle bounding and skipping to condition bone to impact

Strength work takes time to build resilience. Most runners see reduced injury risk after 8–12 weeks of consistent, progressive strengthening.
